About Us
We are dedicated to providing a seamless connection between maternal patients and healthcare services. Our goal is to ensure patients can easily access the care they need when they need it, regardless of what barriers they may face.

Who We Are
Maternal Child Health at Home (MCHH) is a Population Health Care company that focuses on improving quality and health outcomes of the maternal population. We achieve improvements by providing high quality care and multiple layers of support, within the patient’s home setting.

Our Mission
At MCHH, we strive to bridge the gap in healthcare access for women and children by providing collaborative, multidisciplinary home visits that result in improvements in overall health and well-being of our most vulnerable populations

Background
Maternal mortality and morbidity rates are alarming, especially among black women and Texas is one of many states with significant opportunities for improvement.  With the growing decline in maternal health, many notable initiatives, programs and funding opportunities have been created to help improve maternal mortality and morbidity rates for women who actively seek care.    Â
MCHH has taken an additional, crucial step by offering a unique set of services that are provided to the patient where they are often most comfortable, within their home setting. Our unique approach takes into consideration and eliminates the barriers that frequently hinder maternal health patients from seeking care. Barriers such as lack of transportation to medical appointments, lack of support to watch other children during appointments and stigmas around seeking behavioral health services are mitigated through our home care service.

Services
MCHH takes a holistic approach to understanding and addressing the needs of the maternal population. Our care team of experienced Clinicians including, Physicians, Nurse Practitioners (NPs), Registered Nurses (RNs), Doulas, Community Health Workers (CHWs), Behavioral Health Professionals, Lactation Consultants, Medical Assistants (MAs) and support staff work seamlessly to address the unmet needs of the maternal population, all within the comfort of the patient’s home. Our care team serves as an extension of the patient’s primary provider and collaborates to ensure achievement of the best outcomes for patients who are enrolled in one of our specialty programs:
